Click the "Submit Anonymous Feedback" button on the welcome page. You don't need an account. You'll receive a reference number to track your submission later.

Can I attach files to my feedback?

Yes, the anonymous form allows you to upload images or documents (max 16MB total).

How can I check the status of my feedback?

Use the "Track Your Feedback" button on the welcome page. Enter your reference number and the contact info you provided (if any).

Is my identity really anonymous?

Yes, unless you voluntarily provide your email or phone number in the optional fields, we collect no personal data. Your IP address is not stored.

Who sees my feedback?

Only authorized hospital management and the Health Promotion Unit. It is used for quality improvement and monthly reporting.

How long does it take to resolve a complaint?

We aim to acknowledge complaints within 48 hours. Resolution time varies depending on complexity; you can track progress with your reference number.

I forgot my reference number. What can I do?

If you provided an email or phone, you can request a reminder by contacting the Health Promotion Unit directly. Otherwise, we cannot retrieve it due to anonymity.
